Evaristo and Marieliese Saldana Evaristo, and located at lot 10, block S-12, Diliman estate subdivision, PSD-7365, No. 27 Scout Madrinan street, Barangay South Triangle, Quezon City, Metro Manila, as applied for by Marieliese Saldana Evaristo, co-owner, subject to the conditions prescribed under Quezon City Ordinance No. SP-56, S-93, and to the pertinent provisions of Quezon City resolution No. Sp-808, S-97
